MARAC Referral and MASH Enquiry Form for Domestic Violence

Please see the new combined ‘MARAC Referral and MASH Enquiry’ form for Devon along with the Guidance on how use it.

This form needs to be completed for every high risk DV case.

If there are children in the household then both the MARAC and the MASH part of the form has to be completed and a copy of the whole form has to be sent to both MARAC and MASH.  It is the responsibility of the referrer to send to both MARAC and MASH

If there are no children then only the MARAC side should be completed and sent to MARAC Only

PLEASE NOTE that downloadable copies of both the combined ‘MARAC Referral and MASH Enquiry’ form and the Guidance can be found on the MARAC page of the DCC DSVA website: https://new.devon.gov.uk/dsva/information-for-professionals/marac/ .
